If you are an:

Aegon Retirement Choices or One Retirement customers should complete a Death benefit nomination form (PDF - 148kb).
Other customers (except Retiready) should complete a Death benefits nomination form (PDF - 123kb).
Retirement Control opened before December 2010 should complete the appropriate section of the Retirement Control plan review form (PDf - 174kb).
Retirement Control opened after December 2010 should complete the appropriate section of the Retirement Control plan review form (post '10 version) (PDF - 187kb).
